Onboarding form for new employees.
  • Note: If possible, please give us 2-3 weeks notice so that we can dedicate the appropriate resources to this request. We understand this is not always possible, so we will do all that we can to accommodate your request in a timely manner.
    When a new employee replaces a former employee, email forwarding from the previous email address is a nice way to ensure people aren't left hanging when emailing the old address.
  • If you selected "Yes" above, emails from this email address will be forwarded to the new employee's email address.
  • Examples: sales@myorganization.com, team@myorganization.com, marketing@myorganization.com
  • Please be as specific as possible. Example: The entire "F" drive and the "Marketing" folder on the "Z" drive.
  • Please be as specific as possible. Example 1: User should have access to our development software, but should have "view" rights only. Example 2: User should have access to our development software with the same exact rights as John Doe.
  • Please list any thing else we should know with respect to this user. Examples include: - This person will need multiple monitors at their desk - We will need a docking station for their laptop - This person will need a scanner (Model, if known) - This person will need an office phone (Include specific details such as desired extension & hunt groups)
